The Billing Specialist is responsible for performing the following duties:

• Reviews charges, payments and adjustments to ensure accuracy and compliance with billing, coding and any contractual requirements

• Generates patient statements and collection letters
• Enters payments received from payers and patients in the practice management system

Duties:

Care Coordinator is responsible for facilitating transitional/post-acute services for patients. Care Coordinators will work in office, or field settings supporting our Clinicas del Camino Real membership. They support the patients through timely, high quality, and efficient health care which includes all transitional/post-acute support services and through the comprehensive adherence to the multi-disciplinary care plan, patient self-management goals and referrals to both internal and external resources.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
The Care Coordinator is responsible for providing assistance to the care management team by working collaboratively to support patient care plans and needs. Serving as an advocate for patients, the Care Coordinator will be part of the care management team and work in tandem with care managers, healthcare providers, community health workers, and community-based organizations to improve outcomes for patients they serve. Working within his/her scope of practice, this role coordinates between health care services, recognizing the holistic needs of the patient, inclusive of patient specific social and cultural dynamics. The Care Coordinator will support CDCR goals and objectives in meeting performance improvement targets, meeting expectations of standardizing the plan of care, and supporting team development.
• Implements and supports Care Management interventions per the patient’s care plan or assessed community needs
• Coordinates Case Management and Transition of Care referrals and obtains UM authorizations in accordance with member’s needs in a timely manner.
• Performs post discharge outreach calls and scheduled interval follow up outreach calls to support care plan planned interventions.
• Documents all interactions with patients/others appropriately in the care management software.
• Schedules/verifies appropriate medical appointments for patients as needed.

Under the general supervision of the Medical Management Director, the Enhanced Care Management Nurse (ECM) provides efficient and effective Care Management to qualified members by addressing the member’s medical, physical, and psychosocial needs. Using clinical judgement and critical thinking skills, the ECM Nurse develops, modifies and assists the member to implement their care plan and ensure all tasks are timely and in compliance with time requirements per Model of Care/Health plan/DHCS regulatory requirements.
 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

The ECM Nurse’s responsibilities include, but are not limited to:

• Comply with organization and department policies & procedures and Model of Care to ensure all appropriate services for the eligible population are completed and acted upon within established timeframes.

• Manage a case load of 50 Medi-Cal members with complex needs.

Duties: The Health Educator I will be under the general direction of the Health Education Coordinator designing, developing, and implementing Health Education programs and materials for the community clinic. Responsible for developing and implementing a marketing program to promote the organization’s services. Compiles and prepares monthly and quarterly reports to various funding programs. Assists in the in-house training of staff updating education programs and teaching skills. The Medical Assistant is responsible for performing the following duties:

    Interviews patients to obtain medical information and measures their vital signs. 
    Escorts patients to exam rooms and prepares for the medical provider.
    Documents patients’ medical history, vital statistics and information such as test results in the electronic health records (EHR) system.
    Collects blood, tissue or other laboratory specimens as requested by medical provider and prepares them for testing.
    Prepares and administers medications as directed by medical provider.
    Translates and explains treatment procedures, medications, diets and medical provider’s instructions to patients.
    Performs blood draws, administers immunizations and injections.
    Prepares treatment rooms for patient examinations, keeping the rooms neat, clean and fully stocked.

Under the direct supervision of the Dentist or the Lead RDA, the RDA/RDAEF may perform the following duties:

• Sterilizes, prepares and arranges equipment and accessories.
• Keeps dental department and equipment neat, clean and sanitary.

• Utilizes and understands dental terminology.
• Takes digital dental x-rays.
• Instructs patients in the proper practices of dental hygiene.

• Takes impressions for diagnostic and opposing models.

• Assists with orthodontic procedures such as placing and removing elastic orthodontic separators, taking intra-oral measurements for orthodontic procedures, seating adjusted retainers or headgears, checking for loose bands, removing arch wires and ligature ties.
