Duties

Under the supervision of the Director of Health Education and Prevention Programs, the Health Education Outreach Coordinator (HEOC) is responsible for the day-to-day operation of Health Education enrollment and outreach programs and participates in program planning and evaluation methodologies. The HEOC coordinates outreach and enrollment efforts into CDCR programs and medical health plans to both the existing and potential CDCR patient population, residents of Ventura County and the CDCR community at large.  Establishes relationships with other health care agencies and programs for coordination of public health education. Coordinates and trains staff to Health Education Department priorities and processes, implements Health Education programs from both within CDCR and the Ventura county community. Interacts with a multi-cultural population of diverse socioeconomic backgrounds, levels of education and ages.

Requirements

-Community Outreach Health Worker I: One year or less of work experience

-Community Outreach Health Worker II: Two or more years of experience as a Community Outreach Health Worker I

-Community Outreach Health Worker III: Three or more years of experience as a Community Outreach Health Worker II

-Senior Community Outreach Worker: Four or more years of experience as a Community Outreach Health Worker III

 

Must be a graduate from an accredited High School or GED Certificate. Ability to implement outreach presentations and function effectively as a member of the Health Education and Prevention Programs team. Ability to speak English and Spanish and communicate clearly orally and in writing. Must possess a valid California Driver’s License and have reliable transportation with appropriate insurance coverage. Other duties as assigned by the incumbent’s supervisor.
